Common Ground provides access to a continuum of low-barrier, high-tolerance services for people living with HIV. We make access easy and we understand that clients often face other problems like poverty, addiction, homelessness and mental health issues.

Our Case Management approach provides linkages, resources, referrals and support for the different challenges our clients face. A weekly Treatment Team Meeting is devoted to coordinating each person’s care to assure their ability to access needed services.

  • Physical care: We help clients book doctors and dentist appointments, get medications, find an HIV-friendly pharmacy and enroll in drug and alcohol treatment. We make sure they have food, understand nutrition on a budget, and help arrange transportation to appointments.

  • Mental health: We offer innovative services like curbside counseling and drop-in anytime groups as well as traditional counseling to support goals such as reducing isolation, increasing coping skills, managing disorders and getting drug treatment.

  • Housing and employment: We connect people with appropriate housing, whether it’s residential drug treatment, an emergency transition or permanent housing and we provide links to financial assistance, tenant advocacy, job programs and other supportive services.

All of these services are delivered to our HIV positive clients at no cost, and all it takes to become a client is a simple call or visit to our community center.
